West Toronto Youth Hub Official Launch

We are thrilled to invite our community partners, youth, young adults and caregivers to the official launch of a new Youth Integrated Care site at our West Toronto Youth Hub on Tuesday, May 7th from 4:30 pm-6:30 pm.
Come visit our integrated care location and see how mental health services, social services, and primary health care along with multiple organizations are working together. Our collective objective is to provide the best possible outcomes for youth and young adults through a developmentally appropriate and evidence-informed approach. We will also acknowledge the official launch of CHOICES, a substance misuse prevention and intervention program.
Join us as we celebrate with presentations by youth, staff and community members.
